Please complete this form to nominate an appropriate candidate for a position on the Board of Directors that is up for election in the upcoming AGM.

Please complete the information below, with regards to the person being nominated.

By completing this form, you acknowledge that the nominee is in agreement with this nomination, and meets the eligibility requirements to be a member of the board of directors:

8.1.1 Be at least eighteen (18) years of age;

8.1.2 Not be an undischarged bankrupt or of unsound mind;

8.1.3 Be a Member in good standing of the Association or a member of one of the Centre Participants
(as that term is defined in their respective bylaws or constitutions, as the case may be);

8.1.3(.b) Be a current or previous member of the Niagara North Board of Directors or a member of the Board of Directors of one of the Centre Participants (Position of President only)

8.1.4 Remain a Member in good standing of the Association throughout his term of office; and

8.1.5 Shall receive a clear police screening report from the police force having jurisdiction where the
Director resides.
