The following forms need to be completed by player/parents and returned to coaching staff as soon as possible:
Niagara North AAA Zone Hockey Association Player Parent Code of Conduct
HDCO Medical Information Form (first two pages only - remaining forms relate to injury reporting for coaching staff)
Please complete both of these forms and return to Nikki as soon as possible.
A Privacy Policy (which will provide consent to posting of roster information & photographs from events/tournaments) is being developed - check back later for the link to the form.
